your intake form before your appointment.

Provider Arrives
On the confirmed date and time, your assigned provider will arrive
to deliver your IV therapy service.
Important Notice!
⚠️ Good Faith Exam (GFE) Requirement
All patients are required to complete a Good Faith Exam (GFE) virtually prior to their first scheduled appointment.
- The GFE only takes 3–5 minutes and costs $25.
- It is valid for 1 year — if you rebook within that time, you won’t need to complete another.
- The screening is done virtually and fulfills state-mandated health requirements for safe IV therapy.
- During the GFE, you’ll answer questions about your health history and current needs.
- You’ll receive a link to complete this GFE after your provider has been assigned.

About The IV Society
Exam Details:

Regulatory Compliance: This screening fulfills state-mandated health requirements, ensuring all IV therapies are delivered in a safe and responsible manner.
Personalized Care: The evaluation allows our providers to review your health history and current needs, so each IV treatment is customized for your safety, comfort, and optimal results.
When Required:
Prior to your first IV appointment scheduled on or after September 1, 2025 (renewed annually)
